We assist individuals in transitioning from agency services, toward adaptive behavior and independent living mastery.

OUR FAMILY COMMUNITY SUPPORT SERVICES
Our community support services team assists the families of children and adults with development disabilities and severe emotional disturbances (DD/SED). We strive to improve the basic functioning of these individuals through enhancing their capacity for daily social interaction, and improve social functioning, in areas important to client needs, maintaining or reestablishing residency within their community, and in reducing personal distress. Focus areas include work and peers groups reintegration.
HOME BASE MENTAL HEALTH SERVICES
Crown Medical/ Mental Health Service practitioners provide in-home support and respite relief, in support of the families we serve. Our community commitment to personalize treatment governs the approach of our home-based team. Our direct care experienced staff are committed to therapeutic support, family development, and maximal emotional adaptability.
Crisis intervention services essential to personal stability and restoration of individual social functioning are conducted within a client-centered, least restrictive environment. Client aims and treatment satisfaction is objectively calculated, in assuring the attainment of internal benchmarks, and quality, for which the clinic is widely known.
The typical therapeutic process includes an individual history, objective evaluation of reported symptoms, and a comprehensive care plan. Our integrated service model allows for swift recovery, desired grains, sustained progress for individual families and the community we serve.
Josephine (JO) Nwoke is an LICSW. She has a Master’s degree in social work practice with emphasis on families and children. She has more than and mental health services to individuals, groups and families with emphasis on psychotherapy, chemical dependency utilizing the DSM –IV multi-axial systems and dealing with domestic abuse issues. Her other areas of expertise include child protective services, case management, provision of assessment and evaluation for IEP/emotional behavioral disorder, crisis intervention and linkages of individuals and families to appropriate community support & treatment.
.SUMITRA L. DORNER, MSW, LICSW.She has a Master’s degree in
social work practice with emphasis on families, women, and
children. She has many years of experience providing
diagnostic assessments and behavioral health services to
individuals, groups and families. Her other areas of
experience include adoption, case management, assessments,
parenting, relationships, chemical dependency issues, healthy
lifestyles, life coaching, evaluation for IEP/emotional
behavioral disorder, general mental and physical health, and
crisis intervention
